Clinical use of Video Head Impulse Test in Patients with Vertigo in a Neurotologic Clinic

Video Head Impulse test is a well-established test in the diagnostics of vertigo, which allows clinicians to evaluate all semicircular canals (SCC) and its vestibulo-ocular reflex (VOR) pathways independently. Several studies have evaluated the clinical efficiency of video head impulse tests (vHIT) among individuals with peripheral and central vestibular disorders. However, none of them have compared the usefulness of vHIT and its findings across various vestibular disorders. The study aims to estimate the prevalence of vHIT abnormalities across several peripheral and central disorders of the vestibular system in a neurotological clinic. A prospective study was conducted among 100 patients with various vestibular disorders attending neuro-otology clinics between the age range of 18 to 80 years. All patients underwent case history, otoscopic examination, vestibular function tests, routine audiological evaluation, and laboratory vestibular evaluation to identify and diagnose peripheral and central vestibular disorders. Further patients indicative of central vestibular disorders was subjected to neurological and radiological evaluation as needed. vHIT was carried out on all patients in initial or follow-up visits. Each semicircular canal VOR gains, covert and overt saccades, and asymmetry ratios (between the right and left canals) were analyzed. Out of 100 patients, central vertigo was diagnosed in 29 patients, and peripheral vertigo in 71 patients. Fifty-three patients (central vestibular disorders − 6, peripheral vestibular disorders − 47) showed vHIT abnormality. The vHIT abnormalities were more common in peripheral vestibular disorders than central vestibular disorders. Interestingly, three patients showed isolated canal dysfunction despite normal VEMP test findings. The vHIT is an easy-to-use, portable and effective tool for assessing the semicircular canal and VOR pathway, especially in acute vestibular syndromes. Also, as a part of the vestibular test battery, it is useful in the differential diagnosis of peripheral vestibular disorders and central vestibular disorders.
