<p>Pleomorphic dermal sarcoma is a rare skin malignancy with non-specific clinical presentations. Histology and immunohistochemical examinations are both required for an accurate diagnosis, while surgery remains the gold standard of treatment. We report a 53-year-old man who presented with more than 20 years of swelling over the left ear pinna. An initial impression was a keloid; however, the lesion did not respond to treatment, hence incisional biopsy over the left pinna lesion and fine needle aspiration of left neck swelling was made. The diagnosis was then revised to an aggressive malignancy which subsequently requires extensive surgery. This case report emphasizes the importance of attention in atypical dermatological presentations.</p>
