Efficacy of Mobile Application in Comparison to Pure Tone Audiometry in Diagnosing Hearing Loss

Hearing loss affects over 1.5 billion people globally and poses emotional, social, and economic burdens. Access to standard audiometry is limited in resource-constrained settings. Smartphone-based applications offer a potential alternative for hearing screening. We aimed to assess the agreement and diagnostic accuracy of a smartphone-based hearing assessment application in comparison with pure tone audiometry (PTA), the gold standard for hearing evaluation. A cross-sectional descriptive study was conducted on 60 patients (120 ears) aged 16–50 years presenting with hearing loss at a tertiary care center in Northern India (May 2024–May 2025). All participants underwent hearing testing using both PTA and a smartphone-based application, JSS Shravan Mitra. The mean hearing threshold was significantly lower with the mobile app (29.34 ± 14.09 dB) compared to PTA (33.91 ± 17.61 dB; p = 0.001). A strong correlation was observed between both methods (r = 0.826; p < 0.05). Moderate agreement was found in hearing loss classification (κ = 0.452) and overall detection (κ = 0.571). The smartphone application showed 84.7% sensitivity, 74.3% specificity, 88.9% PPV, 66.7% NPV, and 81.7% diagnostic accuracy. Smartphone-based hearing applications demonstrate promising diagnostic accuracy and strong correlation with PTA, supporting their use for initial screening, especially in rural and underserved regions. Further validation through larger studies is recommended.
