Excision of vocal fold lesions and narrow band imaging: a comparative study

Surgical methods—Coblation, Radiofrequency (RF), and Laser-assisted excision, are used to treat benign vocal fold lesions (BVFL). Current study aims to compare the efficacy and outcomes of these three surgical procedures with respect to intraoperative bleeding, duration of surgery, and postoperative voice quality in patients.
Materials and methodsA randomized comparative single-blinded study was conducted at PGIMER Chandigarh from January 2023 to June 2024. Ninety patients with BVFL were randomly divided into three groups of 30 each: Group A (RF-assisted excision), Group B (Coblation-assisted excision), and Group C (Laser-assisted excision). Preoperative and postoperative evaluations were performed using Narrow Band Imaging (NBI), Videolaryngostroboscopy (VLS), Voice Handicap Index(VHI) and Multidimensional Voice Program (MDVP).
ResultsSignificant improvements in voice quality were observed in all three groups postoperatively. Coblation and Radiofreqency showed shorter operative times compared to LASER, with mean durations of 7.78 and 7.08 min respectively, versus 9.39 min for LASER (p = .210). Voice Handicap Index scores improved significantly, with 12-week postoperative mean of 4.37 for Laser, 4.23 for Coblation, and 2.60 for Radiofrequency (p = .246). No significant differences were found in intraoperative bleeding among the three modalities. NBI findings correlated well with the post-surgical histopathology.
ConclusionAll three surgical modalities—Coblation, Radiofrequency and LASER—are effective in excising BVFL, with each showing significant improvements in voice quality and minimal complications. Coblation and RF, with their shorter operative times, present as viable alternatives to Laser excision.
