<p>There is a shortage of studies comparing the complications of formalizing tracheostomy techniques with open tracheostomy incisions. Ovid MEDLINE, EMBASE, CINAHL, and Cochrane Central Register of Controlled Trials (CENTRAL) databases were searched. Studies that evaluated complications in adults who underwent open tracheostomy using an alternative open surgical technique with or without comparison to conventional open surgical technique were included. Literature search identified 896 studies, of which 7 were selected for systematic review, with 1837 patients who underwent formalized tracheostomy technique and 121 patients who underwent conventional open tracheostomy. Short-term complications for the alternative group and conventional group include bleeding (3.1% versus 1.0%, respectively), infection (1.9% versus 2.0%), and tube extrusion (0% versus 3%). Long-term complications included tracheal stenosis (0.86% versus 5.0%) and trachea-cutaneous fistula (3.2% versus 0%). This review shows no increased complication rates in alternative open tracheostomy compared to conventional tracheostomy techniques.</p>
