Transcripto-Proteomic Analysis of Glucocorticoid Receptor Alpha Isoform in Oral Squamous Cell Carcinoma, Oral Sub Mucous Fibrosis, Oral Lichen Planus and Oral Epithelial Dysplasia

Approximately 90% of malignant growths in the head and neck area are attributed to Oral Squamous Cell Carcinoma (OSCC). The advancement from Oral Potentially-Malignant Disorders (OPMDs) to malignancy involves multiple factors, with GRα (a variant of the glucocorticoid receptor) playing a role as a transcriptional regulatory element. More GR alpha expression is proven to indicate less aggressive malignant behavior. Studies have shown that overexpression of GR accelerates tumor cell death and has a significant role. We aimed to evaluate the asses the presence of glucocorticoid receptor alpha (GRα) isoform in OSCC, OSMF, OED, and OLP. Hundred and five tissue samples from a diverse array of clinically confirmed cases of OSCC, OSMF, OED, and LP, along with samples of healthy oral mucosa (control tissue) were taken. Biopsies were collected from the respective oral sites, and they were preserved in RNAlater (for qRT-PCR Study) and 10% formalin (for IHC study). In both the IHC and qRT-PCR analyses, the levels of GRα in lesions of OLP, OSMF, OED, and OSCC were significantly reduced (< 0.05) in comparison to the healthy control group. Notably, normal oral mucosa exhibited the highest GRα expression. Furthermore, the level of GRα expression appeared to be decreased in patients with OLP compared to those in a healthy control group, a trend observed similarly in individuals with OSMF and OSCC. The data provided compelling evidence that GRα significantly contributes to the anti-inflammatory response linked to the pathogenesis of OLP, resulting in reduced cytoplasmic and nuclear expression. Moreover, the notably lower expression in OSCC indicates that GRα may undertake distinct roles within the context of OSCC.
