Comparison of Lymph Node Yield in En Bloc v/s Level Wise Lymph Node Dissection in Management of Head and Neck Squamous Cell cancers: A Prospective Randomised Study

Lymph node yield is an important factor in management of head and neck squamous cell carcinoma (HNSCC). It is one of the factors that helps in deciding prognosis and planning of adjuvant therapy. Different techniques in lymph node dissection have been studied with their respective outcomes. We have compared enbloc and level wise neck node dissection in terms of their lymph node yield. Our study is single centre, hospital based, prospective randomised study in which we have included total 30 patients and randomized to level wise level versus enbloc lymph node dissection technique in HNSCC cases operated between June 2023 to June 2024 at National Institute of Medical Science and Research, Jaipur. Out of 30 patients, 16 patients were in level wise lymph node dissection group (Group A) and remaining 14 patients were in en bloc lymph node dissection group (Group B). Lymph node yield results were stratified based on different variables that could impact the outcome. Our study is still going on with recruitment of more patients in the study. The mean lymph node yield in Group A was 34.06 ± 13.16 while it was 23.92 ±10.88 in Group B. The difference in lymph node yield in 2 groups was statistically significant (p <0.05). Level wise lymph node dissection technique yields more number of lymphnodes as compared to en bloc lymph node dissection. The data on whether the difference in lymph node yield translates into difference in oncological outcomes will be available after a follow up period and can be published as a sequential result to this study.
