<p>The incidence of submandibular salivary calculi is approximately 1.2%, with the majority located in the duct. Parenchymal calculi are less commonly reported and typically present acutely with pain or complications. The submandibular gland calculi occur more frequently than those in the parotid gland due to various anatomical and physiological factors. These factors also predispose to a higher incidence of ductal rather than parenchymal calculi. Calculi exceeding 35&#xa0;mm in diameter are classified as ‘giant’. Reports of parenchymal calculi, particularly asymptomatic giant forms, are uncommon. To date, only one similar case has been published, involving a calculus with a maximum diameter of 30&#xa0;mm. Here we report a case of right-sided submandibular gland parenchymal calculus in a 67-year-old man presenting with a painless swelling. The calculus measured 4&#xa0;cm * 3&#xa0;cm in size. Excision of the right submandibular gland was performed via cervical incision. Postoperative histopathology indicated chronic inflammation with acinar damage in the residual gland tissue. This case represents the largest reported asymptomatic submandibular parenchymal calculus to date.</p>
