Hearing Screening for Children of School Age

Hearing disorders in school-age children can negatively impact speech, language development, academic performance, and social integration. Although pure tone audiometry remains the gold standard for hearing assessment, its reliance on specialized equipment and soundproof rooms limits its availability, particularly in low-resource settings. Web-based hearing screening provides a low-cost, easily deployable alternative for the initial detection of hearing impairment. To describe the hearing characteristics of school-age children undergoing web-based hearing screening at a secondary-level hospital in Ponorogo, Indonesia, and to compare the screening outcomes with conventional pure-tone audiometry. A retrospective descriptive study was conducted using secondary data from 101 children aged 6 to 18 years who attended Yasyfin Gontor Hospital on August 7, 2024. Inclusion criteria included cooperative children with complete demographic data, while exclusion criteria comprised uncooperative behavior or anatomical limitations to audiometry. All participants underwent web-based hearing screening (35 dB, 55 dB, 75 dB across 500 to 8000 Hz), followed by pure tone audiometry to assess air-conduction thresholds. Otoscopic examination findings and the anamnesis of hearing complaints were also documented. The cohort included 44 males (43.6%) and 57 females (56.4%), with 73% between the ages of 6 and 8 years. Anamnesis revealed no subjective hearing complaints among all participants. Otoscopic examination indicated cerumen presence in 19.8% of the cases and intact tympanic membranes in 81.2%. Web-based screening demonstrated normal hearing in 99% (100/101) of ears at 35 dB; one child (0.9%) required intensities of ≥ 55 dB across multiple frequencies. Pure tone audiometry corroborated these findings, identifying a single case of mild hearing loss (AC > 25 dB) and showing 99% concordance with the web-based test among the other participants. Web-based hearing screening demonstrated high concordance with pure tone audiometry and effectively identified hearing loss in school-age children. Given its simplicity and minimal infrastructure requirements, web-based screening can serve as a practical alternative for large-scale school health programs in resource-limited settings.
