Mini Clinical Evaluation Exercise (Mini-CEX) as a Formative Assessment Tool in ENT Residency: A Pilot Study

The Mini Clinical Evaluation Exercise (Mini-CEX) is a brief (10–20 min) structured assessment of a doctor patient interaction. It is designed to evaluate clinical skills, attitudes, and behaviour, critical for delivering high-quality patient care. Mini-CEX aligns with the highest level of Miller’s pyramid “Does” which reflects actual performance in clinical settings. The aim of the study is to implement Mini-CEX as a formative assessment tool in the Department of otorhinolaryngology to evaluate the perceptions of both faculty and postgraduate (PG) students regarding its utility. A prospective interventional study was conducted in the Department of Otolaryngology at a tertiary care centre Hospital for a period of 6 months. The study included 8 first year and 4 s year postgraduate students, assessed by 4 faculty members. Each resident participated in three Mini-CEX sessions involving different clinical cases. Scores from the first and last sessions were recorded and compared. Perceptions of students and faculty were collected at the end of the sessions using a 5 point Likert scale. There was a statistically significant improvement in Mini-CEX scores from the first to the final session (p = 0.0123). Second-year residents showed greater improvement, suggesting their enhanced ability to utilize prior clinical experience and feedback. Perception surveys indicated a positive response from both students and faculty regarding the effectiveness in improving clinical performance. Mini-CEX proved to be an effective formative assessment tool in ENT, facilitating consistent feedback and measurable improvement in clinical competence. Given the positive outcomes and feasibility demonstrated here, we recommend the structured integration of Mini-CEX into the ENT postgraduate curriculum as a regular formative assessment tool. The positive perceptions of both faculty and trainees further support its implementation in postgraduate medical education & improve their clinical skills in diagnosis.
