<p>To assess the real-world impact of the COVID-19 pandemic on short-term audiological and speech outcomes in children with cochlear implants, especially focusing on disruptions in follow-up care. This retrospective cohort study was carried out at the Department of Otorhinolaryngology, Sardar Patel Medical College, Bikaner, India, covering the period from May 2018 to January 2022. We included 100 prelingual deaf children under 6 years who received cochlear implants and completed 12 months of follow-up. Children were categorized into Group A (operated and followed up pre-COVID-19) and Group B (operated before or during the pandemic but followed up during the COVID-19 era). Auditory and speech outcomes were evaluated using CAP, SIRS, and MAIS. Statistical analyses were performed using SPSS v26.0. Group A demonstrated better outcomes in CAP ≥ 7 (76% vs. 42%), SIRS scores 4/5 (84% vs. 64%), and MAIS “Good” ratings (88% vs. 62%), all with statistically significant differences. Irregular follow-up, more prevalent in Group B (76% vs. 12%, p &lt; 0.001), was associated with poorer outcomes. Positive correlations were found between regular follow-up and higher CAP, SIRS, and MAIS scores. Interruptions in post-implant rehabilitation due to the pandemic adversely affected outcomes. The study emphasizes the need for robust tele-rehabilitation strategies to sustain care during healthcare disruptions.</p>
