<p>The report discusses the case of a 16-year-old girl diagnosed with tubercular osteomyelitis of the mandible. She presented with a one-month history of swelling in her right cheek, a past extraction of the third molar under local anesthesia, and a history of sinus drainage. Patient reported with complaint of non healing of extraction socket, swelling and complaint of weight loss following which we suspected tuberculous infection and advised some test and biopsy of the tissue for definitive diagnosis we came positive for tuberculous osteomylitis. Following the initiation of anti-tubercular therapy, the patient showed significant improvement during follow-up. Chronic maxillofacial infections unresponsive to standard antibiotics and local interventions should prompt consideration of a possible mycobacterial infection. This case aims to highlight the atypical presentations of TB (Tuberculosis), emphasizing the need for timely diagnosis to avoid unnecessary investigations and inappropriate treatment. This case report aims to raise awareness among healthcare providers about the unusual presentations of TB, which, if missed, could lead to unnecessary tests and incorrect treatment.</p>
