Current Trends in Neoadjuvant Chemotherapy in Sinonasal Malignancies: A Scoping Review

Sinonasal malignancies comprise a small subset of head-neck malignancies, but typically present at an advanced stage and have a propensity to invade adjoining critical structures like the eye and the brain. Radical surgery is the mainstay of treatment but often it leads to significant morbidity. This review aims to investigate the role of neoadjuvant chemotherapy (NACT) in the management of sinonasal malignancies with special emphasis on survival outcomes and organ preservation. PubMed, Scopus. The databases were searched for relevant articles on the role of NACT in sinonasal malignancies. Articles in the last ten years and English language articles were selected for review. 12 articles were selected for the review. Most common indication of giving NACT was advanced T stage (T3, T4). Survival was significantly better in patients who responded to NACT compared to those who did not. Chances of orbital preservation were more in patients who responded to NACT than in patients who did not. NACT appears to have a significant effect on the survival of patients with sinonasal malignancies. Effect on orbital preservation is less robust. Since the studies in question were retrospective reviews, more high-powered randomized trials are required to further investigate the role of NACT in this subgroup of patients.
