<p>Though papillary and follicular carcinomas are individually common thyroid malignancies, concurrent follicular-papillary lesions are rare, where two histologically-distinct synchronous primary neoplasms coincide at the same location. We are reporting a thyroid collision tumour combining papillary and angio-invasive follicular carcinoma which was diagnosed postoperatively via histopathology. The case highlights diagnostic challenges due to unclear preoperative indicators, and the need for individualized treatment. Surgical excision, radioiodine therapy, and thyroxine suppression led to a disease-free outcome for 8 years, emphasizing multidisciplinary management.</p>
