<p> Hearing impairment acquired in the pre-lingual period affects auditory development, leading to deaf-mutism (DM). Despite its high prevalence in India, no standardized tools exist to assess pre-, peri-, and post-natal risk factors. This study aimed to develop and validate a Hindi questionnaire for systematic risk factor assessment in individuals with severe-to-profound congenital hearing loss. An exploratory sequential study design with an instrument development model was used. The validation process included five phases: content validation by 10 experts (I-CVI = 0.90, S-CVI/Ave = 0.90), reliability testing with 50 participants (Cronbach’s Alpha = 0.75, ICC = 0.75), construct validity assessment through factor analysis, pilot testing among 73 participants (90% clarity, average completion time: 15–25&#xa0;min), and large-scale validation with 600 participants following IEC approval. The questionnaire was found to be a reliable and valid tool, with factor analysis identifying four key domains explaining 90% variance: pre-natal (30%), peri-natal (25%), post-natal (20%), and socio-demographic (15%). The Kaiser-Meyer-Olkin (KMO) measure was 0.85, and Bartlett’s Test (<i>p</i> &lt; 0.001) confirmed the suitability of factor analysis. Large-scale validation reinforced its generalizability and usability. The validated Hindi questionnaire is a reliable tool for assessing risk factors for severe-to-profound congenital hearing loss. It is feasible for clinical and research use and could aid in nationwide epidemiological studies. Future research should focus on expanding validation to other linguistic groups and integrating it into national neonatal screening programs for early intervention.</p>
