<p>A retrosternal goitre is generally defined as a thyroid mass in which 50% or more of its volume is located below the thoracic inlet. This can also be called as intra thoracic goitre. Retrosternal goitre is a very rare condition which account for 5 to 20% of thyroidectomy patients.</p><p>We present the case of a 52-year-old female who presented to the emergency department with acute onset dyspnoea and chest pain, raising initial suspicion of an acute coronary syndrome. On Further investigation, revealed a massive retrosternal goitre as the underlying cause of her symptoms. She underwent thyroidectomy with sternotomy, with complete resolution of symptoms after surgery.</p><p>This case highlights the importance of considering non-cardiac aetiologies, when evaluating chest pain and dyspnoea.</p>
