Anthropometric Analysis of Base of the Nose in an Indian Population: A Critical Analytic Propositions

The nose types are determined by the shape of the nostrils, variations in soft tissues, and the bony contours of the nose, which result in distinct, consistent nasal subunits. This study emphasizes on the base of the nose and its overall impact on the aesthetic outcome which includes nasal breadth, alar length, symmetry of the nostril, columellar length, columellar width, columellar show, and nasal angles. It can be quite difficult to achieve optimal aesthetic and functional outcomes in rhinoplasty without a systematic approach to the base of the nose. This study thus provides credible and objective reference material to the surgeon for the external nasal soft tissue evaluation and for planning rhinoplasty. Prospective observational study conducted in the Department of ENT at a tertiary care hospital from October 2022 to September 2023 which included people from North to South India attending ENT OPD, medical students, postgraduates, nursing staff and paramedical staff within age group 18–55 years attending ENT OPD and excluded subjects with previous history of orthodontic treatment, history of facial plastic surgery, trauma to orofacial region, history of temporomandibular joint disorders, systemic syndromes and craniofacial abnormalities. Photographs were taken using DSLR camera and measurement of nasal parameters was done with digital sliding vernier calipers. Nasal angles were measured from photographs using ImageMeter-photo measure application. Students unpaired t test and Chi-square test were used to compare the nasal parameters based on gender and region of India. This study included 141 subjects in the age group of 18–55 years among which 45% were males and 54% were females. Results were analysed using SPSS software. Males have greater nasal breadth, alar length, and columellar width than females. North Indians had significantly more nasal breadth, alar length and columellar width compared to south Indians. Among 141 subjects 29 had symmetrical and 112 had asymmetrical nostrils. There is a linear relationship between upper third and lower third of columella which was statistically significant. Nasal parameters are different for gender, ethnic and geographical variation. Thus, requiring attention while planning for corrective surgeries of nose. These results can be used to extrapolate reference to plan for rhinoplasty surgeries.
