<p>Pleomorphic adenoma, typically benign and slow-growing, rarely metastasizes without malignant transformation. We report an unusual case of early pulmonary metastasis within a year of submandibular tumor excision. This highlights the need for long-term follow-up, multidisciplinary evaluation, and reconsideration of risk, even in histologically benign salivary gland tumors.</p>
