Cephalometric Evaluation of Upper Airway Dimension and Hyoid Position After Treatment with Functional Appliance

This study aimed to evaluate cephalometric changes in upper airway dimensions and the hyoid bone position after treatment with functional appliances in growing patients and compare them with a control group. This case–control study was conducted on 30 patients treated with functional appliances (12 male and 18 female, mean age: 10.43 ± 1.38 years) and 30 patients (9 male and 21 female, mean age: 11.60 ± 1.32 years) as the control group. Pre- and post-treatment lateral cephalometric radiographs of both groups were evaluated for linear and angular measurements of airway dimension and hyoid position. A comparison between the pre- and post-treatment measurements in the case group showed significant changes in SNB, ANB, Wits, UPA, and LPA landmarks, compared to the control group (P values were 0.02, 0.001, 0.004, 0.001, 0.002, and 0.001, respectively). Also, SNB increased, ANB and Wits decreased, and UPA (upper pharyngeal airway) and LPA (lower pharyngeal airway) increased significantly (P values were 0.001, 0.001, 0.02, 0.001, and 0.003, respectively). The results of the study also showed a significant increase in the post-treatment hypopharyngeal dimension (V-LPW) compared to the control group (P value = 0.00). No significant changes in the hyoid bone position were observed in the treatment group compared to the control group. This study examined the pre- and post-treatment cephalograms of skeletal ClII patients to investigate the effects of functional appliances on the airway dimensions and the position of the hyoid bone. Patients with Class-II malocclusion who were treated with functional appliances showed significant changes in the dimension of their upper and lower airways, but there were no significant changes in the position of the hyoid bone.
