MRI Characteristics and Misdiagnosis of Solid Pleomorphic Xanthoastrocytoma: Case Report and Bibliometric Analysis of Literature

Solid pleomorphic xanthoastrocytoma (sPXA) is a rare variant of PXA that presents diagnostic challenges due to its atypical MRI features. Unlike classic PXA, which typically exhibits a cystic-solid mass, sPXA lacks cystic components, leading to frequent misdiagnosis as high-grade gliomas or meningiomas. In this study, we retrospectively analyzed six pathologically confirmed sPXA cases, focusing on MRI characteristics, histopathology, and clinical presentation. Our findings showed that sPXA most commonly occurs in the supratentorial region, particularly the frontal lobe, with iso-/hypointensity on T1WI, hyperintensity on T2WI, and significant peritumoral edema. Four cases exhibited marked enhancement, including a “sausage-like” pattern along gyral structures. Histopathologically, all tumors featured pleomorphic cells, including spindle cells, multinucleated giant cells, and foamy xanthomatous cells, with a high prevalence of BRAF V600E mutations. Additionally, a bibliometric analysis revealed a lack of dedicated sPXA research, with growing interest in advanced neuroimaging and molecular diagnostics. This study highlights the need for increased awareness of sPXA’s distinct imaging features and advocates for the integration of advanced imaging techniques and molecular diagnostics to improve diagnostic accuracy and clinical management.
