Management of Ear Foreign Bodies: Need to Avoid the Potential Pitfalls

The present study aims to review patients with ear FBs, associated complication and the potential pitfalls leading to such complications.
Materials and MethodsDesign - A retrospective descriptive chart review of patients that were managed for aural foreign body impaction over a six-year period. Setting - Otorhinolaryngology Department of UNIOSUN Teaching Hospital, Osogbo Subjects: All patients with ear FB impaction over six years period. Methodology: Identification of the affected patients were made from records from ENT out-patients clinic registration books, theatre register and ward admission records.
ResultsA total of 259 patients consisting of 142 males and 117 females, age ranged from 1 to 75 years. 56% of ear FBs occurred in children less than 10 years. Many patients (36%) were referred to ORL following failed previous attempts with associated complications Most (89.7%) complications occurred in children. Patients without complications were managed by manual FB removal or syringing. Patients with deeply impacted or middle ear FBs and those complicated with granulation tissues growth were removed under general anaesthesia. Those with otitis externa and those that developed CSOM secondary to TM perforation were managed with aural toileting and topical antibiotics Four patients had myringoplasty using facial lata graft due to failure of closure of the perforated TM.
ConclusionPotential pitfalls causing complications in ear FB management include; removal by non-ENT personnel without adequate experience, delayed presentations, forgotten FBs, improper techniques, aggressiveness, inadequate equipment and uncooperative patients. Raising public awareness on preventive strategies, parental watchfulness, keeping of commonly implicating materials away, educating the general duty doctors on their limitations. surgical removal under general anesthesia should be considered to minimize the risk of complications after failed attempted removal.
