<p>Radiation-Induced Myofibrosarcoma (RIMFS) in the head and neck, is a very rare malignant fibroblastic tumor. The diagnosis of RIMFS can at times be overlooked in view of its late presentation after radiation, rarity, and inconsistent clinical presentation and can only be confirmed using immunohistochemistry (IHC). In this case report, we presented one case of RIMFS, who had earlier developed carcinoma maxilla and had undergone surgery followed by radiotherapy and after a decade developed RIMFS in the sphenoid sinus. The diagnosis was a challenge, but with the aid of thorough clinical inspection, histopathology and immunohistochemistry (IHC) we were able to come to the diagnosis and further treatment.</p>
