<p>Adenoid cystic carcinoma (ACC) is a rare malignant neoplasm primarily affecting the salivary glands, most commonly arising in minor salivary glands and presenting in older individuals. It is characterized by indolent but relentless progression, with potential for perineural invasion, local recurrence, and distant metastasis. We present a case of ACC of the submandibular salivary gland in a 20-year-old female—an unusual age for this tumor—highlighting clinical, histological, and molecular features. The patient underwent excisional biopsy and is currently under surveillance with no evidence of recurrence. This case underscores the importance of considering ACC in young patients and reviews recent advances in molecular pathology and targeted therapy.</p>
