<p>Benign paroxysmal positional vertigo (BPPV) affecting multiple semicircular canals, a rare occurrence, is classified as multi-canal BPPV (MC-BPPV). The defining feature is the presence of vertical and horizontal nystagmus. Publications documenting posterior semicircular canal variants, including short-arm and apogeotropic types, have increased over the past decade. We describe a case demonstrating the concurrent presence of right horizontal semicircular canal cupulolithiasis and left posterior semicircular canal short-arm canalolithiasis, and such an instance is not previously reported in the medical literature.</p>
