<p>Giant parathyroid adenoma refers to a type of parathyroid adenoma weighing more than 3.5&#xa0;g causing primary hyperparathyroidism with varied manifestations.We report a case of giant parathyroid adenoma with retrosternal extension measuring 7&#xa0;cm in highest dimension and weighing 60&#xa0;g, presenting with spinal (L4) tumor. A 62 year old female presented with history of pain in both lower limbs and lower back since 2 months. MRI spine showed an enhancing lytic expansile lesion involving posterior elements of L4 vertebra with significant bony central spinal canal compromise for which she underwent L4 laminectomy with decompression of intradural extramedullary lesion. Histopathology of the lesion was suggestive of giant cell tumour-osteoclastoma. On retrospective evaluation following vertebral tumor excision, patient was found to have severe hypercalcemia and elevated parathyroid hormone levels(1243.5 pg/mL). Ultrasound neck revealed a large left paratracheal tumor posteroinferior to left lobe of thyroid suggestive of parathyroid tumor. 99&#xa0;m Tc-Sestamibi scan confirmed a giant parathyroid adenoma and the patient subsequently underwent surgical excision of parathyroid adenoma.</p>
