<p>The nonrecurrent laryngeal nerve (NRLN) is a relatively rare anatomical variation, affecting only about 0.3–0.8% of individuals undergoing head and neck surgeries. Thyroid surgery is the most common surgery where we encounter NRLN. A 49 years old female presented with a thyroid swelling which on FNAC showed features of papillary carcinoma of thyroid. Intraoperatively well defined, hard mass of right lobe of thyroid with extrathyroidal extension and NRLN was observed. She underwent Total Thyroidectomy with Central compartment neck dissection with nerve resection and primary repair of NRLN. Post operative period was uneventful with mild hoarseness of voice. The NRLN can be a surgical surprise intraoperatively increasing chances of nerve injury. Accurate identification and preservation of the RLN are crucial, and a thorough understanding of its anatomical variations is essential to achieving this goal. In case of injury or resection, repair of nerve should be considered.</p>
