Microbial Profile and Antimicrobial Resistance Patterns in Otitis Externa: A Clinical Study from a Tertiary Care Teaching Hospital

Otitis externa is a frequently encountered condition in otorhinolaryngology, characterized by inflammation or infection of the external auditory canal (EAC). It is commonly caused by bacteria, notably Pseudomonas aeruginosa and Staphylococcus aureus, and may be exacerbated by inappropriate use of empirical antibiotics, leading to antimicrobial resistance. This study aimed to evaluate the microbial profile and antibiotic susceptibility patterns among patients with acute otitis externa attending a tertiary care teaching hospital.
Materials & MethodsA retrospective study was conducted from March 2024 to March 2025 on 50 patients diagnosed with acute otitis externa. Ear discharge samples were collected and cultured on blood agar, chocolate agar, and MacConkey medium. Isolates were subjected to antibiotic susceptibility testing.
ResultsAmong the 50 cases, 70% of isolates were P. aeruginosa, followed by S. aureus (12%) and Enterobacteriaceae species such as Proteus, Klebsiella, and E. coli. P. aeruginosa showed high sensitivity to meropenem (89%), piperacillin-tazobactam (89%), amikacin (87%), and ciprofloxacin (85%), but 58% were resistant to ceftazidime. S. aureus isolates were 100% sensitive to vancomycin and 85% to ciprofloxacin and clindamycin but showed high resistance to penicillin (80%). ESBL-positive E. coli exhibited complete resistance to several antibiotics.
ConclusionP. aeruginosa and S. aureus remain predominant pathogens in otitis externa, with notable resistance patterns. Regular microbial surveillance and rational antibiotic prescription are essential to curb resistance and improve patient outcomes in otologic infections.
