Parotidectomy Assisted by 4K Exoscope (VITOM 3D): A Single Centre 5 Years Surgical Experience

The aim of this article is to present our 5 years surgical experience in parotid gland surgery performed using the VITOM 3D system. This study presents a 5 years (2019–2024) single centre surgical experience during which patients underwent parotidectomy assisted by the 3D-exoscope. Outcomes such as surgical duration, facial nerve deficit incidence, post-operative hematoma and surgical specimen margins were analysed. 70 patients (mean age: 56.1 years; range: 15–88 years) underwent exoscopeassisted parotidectomy, 37 (52.8%) were male and 33 (47.2%) females. Superficial parotidectomy (I–II) was performed in 67 patients (95.7%), while total parotidectomy (IV) was conducted in 3 patients (4.3%). The mean operative time, including anaesthesia, was 133 min (range: 60–230 min). Histopathological analysis revealed pleomorphic adenoma as the most common diagnosis (n = 28). 3 patients experiencing transient postoperative marginal mandibular branch palsy (H-B II), resolving fully within 3 months. VITOM 3D technology enhances precision, ergonomics and teamwork in parotid gland surgery. Despite its high cost and learning curve, it is a promising alternative to traditional tools like microscope or surgical loupes.
