<p>Children with cochlear implants (CIs) face elevated behavioural challenges and parental stress, yet these psychosocial dimensions remain underexplored in low-resource settings. This study aimed to assess behavioural problems in CI children, evaluate parental stress, and examine their associations with CI outcomes. Eleven CI recipients (mean age = 8.95 ± 2.39&#xa0;years) were evaluated using the Children’s Behaviour Checklist (CBCL) and Parental Stress Scale (PSS). CI outcomes included Categories of Auditory Perception (CAP), Speech Intelligibility Rating (SIR), and expressive abilities (Integrated Scales of Development, ISD). Statistical analyses employed Mann–Whitney U tests and Spearman’s correlations. Sixty-three percent of CI children exhibited clinically elevated overall behavioural problems (<i>p</i> = 0.008), significantly exceeding normative data. Parental stress (mean PSS = 42 ± 10.81) correlated strongly with anxiety, withdrawal, social problems, thought problems, and externalizing behaviours (<i>p</i> &lt; 0.05). Expressive abilities (ISD) negatively correlated with most CBCL domains (<i>p</i> &lt; 0.05), unlike CAP/SIR. Behavioural challenges in CI children are closely linked to parental stress and expressive communication deficits. Family-centered interventions targeting language development and caregiver well-being are critical to improving psychosocial outcomes.</p>
