Non-invasive Follicular Thyroid Neoplasm with Papillary Like Nuclear Features (NIFTP): Results of Long Term Follow Up from Tertiary Care Centre in an Area of High Prevalence of Thyroiditis

The introduction of NIFTP aimed to provide a more accurate classification and improve the management and prognosis for patients with these tumors. NIFTP creates doubts in the treating physicians and surgeons about the possibility of missing a papillary thyroid cancer and its consequences. The primary objective was to evaluate the long-term outcomes of patients diagnosed with NIFTP, including recurrence rates and metastasis.
Materials and methodsRetrospective cohort study. Study period was 2017 to 2021 with a minimum follow up of 3 years. Patients diagnosed with NIFTP based on histopathological examination and had undergone a Total Thyroidectomy were included. Patients had a pre operative diagnosis of Multinodular goiter involving both lobes (TR2-4, Bethesda II-IV). At the initial follow up when the histopathology report was available a stimulated Thyroglobulin (sTg), Anti Thyroglobulin antibody (sATg Ab) and TSH tests along with an ultrasound neck were obtained. These tests were then repeated in the follow up.
Results37 patients who full-filled the inclusion were included. There were 32 females and 5 males. The age was between 35 and 55 years of age. Follow up period was three years. Serial USG Neck also showed normal scans with no recurrences in thyroid bed or suspicious neck nodes. Serial Tg/ATgAb did not show elevation in the three years of follow up.
ConclusionOur study suggests that the patients diagnosed to have NIFTP had indeed behaved like having benign tumors. Patients diagnosed with NIFTP after a thyroidectomy need not be followed up aggressively.
