<p>Facial nerve identification is very important during parotoid gland surgeries commonly done for pleomorphic adenoma arising from superficial lobe of the gland. The pleomorphic adenoma is most common benign tumor of salivary glands constitutes around 80%. The intraoperative identification and preservation of facial nerve is a major challenge for young surgeons. Previous some reports described the anatomical variations of facial nerve in relation with retromandibular vein. In our index case, we report an interesting case of a 28-year-old female presented with pleomorphic adenoma arising from superficial lobe of the right parotid gland, had an atypical anatomical facial nerve course having lengthy single trunk, floppy, pushed downward and not present in the plane of faciovenous plane of Patey. Although previous many studies available about the anatomical variations of facial nerve depending up on intraoperative findings, we present one novel anatomical course for surgeons’ deep knowledge and perpetual awareness to prevent facial nerve injury during parotid gland surgeries.</p>
