Neutrophil-to-Lymphocyte Ratio and Platelet-to-Lymphocyte Ratio in Patients with Bell’s Palsy

This study aims to evaluate the prognostic significance of the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) in patients with Bell’s palsy and their association with disease severity and treatment outcomes. This prospective case-control analytical study was conducted over 18 months at a tertiary care hospital in India. A total of 62 patients diagnosed with Bell’s palsy and a control group of 70 healthy individuals were included. Patients were classified using the Facial Nerve Grading System 2.0 (FNGS 2.0) at presentation and post-treatment following 21 days of corticosteroid therapy. Pre-treatment NLR and PLR values were derived from routine complete blood counts (CBC). Statistical analyses, including Mann-Whitney U tests and receiver operating characteristic (ROC) curve analysis, were conducted to assess their prognostic value. NLR was significantly elevated in Bell’s palsy patients compared to controls (p < 0.001), with higher values correlating with worse treatment outcomes. ROC analysis identified an optimal prognostic cut-off value of 4.87 (AUC = 0.897). PLR did not significantly differ between patients and controls (p = 0.509) but demonstrated predictive potential for prognosis (AUC = 0.856, cut-off = 156.14). NLR serves as a reliable prognostic marker for Bell’s palsy, while PLR exhibits moderate predictive value for treatment outcomes. Further large-scale studies are needed to confirm their clinical utility.
