<p>Adenoid cystic carcinoma (ACC) is primarily a salivary gland tumor that rarely involves the respiratory tract. Primary malignant tumors of the trachea are very rare, with an incidence of less than two cases per 1,000,000 people per year, and only 10 to 15% of them are ACCs. We present a rare case of ACC of the proximal trachea in a 68-year-old female patient. The patient initially presented with stridor that was first recognized as a symptom of asthma, but a multislice computed tomography scan revealed a tracheal tumor. She afterwards underwent a tracheostomy, during which a biopsy was performed, establishing the diagnosis of an ACC. Subsequently, a surgical resection with a cricotracheal anastomosis was performed. The postoperative course was complicated by bilateral dysfunction of the vocal cords on the third postoperative day and by coronavirus disease 2019 (COVID-19) infection with bilateral pneumonia on the tenth postoperative day. A combination of ACC of the proximal part of the trachea with postoperative development of bilateral vocal cord dysfunction and postoperative COVID-19 infection makes this case uncommon and instructive.</p>
