<p>We report a case of a 4-year-old female who presented with an acute stroke and a renal infarct. Further diagnostic workup with a transthoracic echocardiogram revealed a cardiac mass, and a subsequent cardiac magnetic resonance imaging (MRI) confirmed the presence of a cardiac tumor. The patient underwent surgical resection of the cardiac mass, and histopathological analysis confirmed the diagnosis of a cardiac papillary fibroelastoma (CPFE). This case highlights the importance of considering CPFE in the differential diagnosis for pediatric patients presenting with embolic events and stroke concurrent with a cardiac mass, despite its rarity in this age group.</p>
