<p>Studies have shown that inflammation plays an important role in the pathogenesis of Myasthenia gravis (MG). The neutrophil to albumin ratio (NAR) stands out as a novel inflammatory biomarker; however, its relationship with MG has not been explored. This study aims to investigate the ability of NAR to monitor disease severity and predict the short-term prognosis in patients with MG. This retrospective study included 134 patients with MG. Multivariate regression and subgroup analyses were used to comprehensively explore the association between NAR and short-term prognoses. A prediction model was then built and visualized using a nomogram. Three approaches were used to evaluate the performance of the proposed nomogram. The NAR was significantly higher in patients with more severe disease conditions at baseline (<i>p</i> &lt; 0.001). After adjusting for confounding factors, NAR was still independently associated with short-term prognosis of patients with MG (OR = 5.376, 95%CI = 1.321–21.881, <i>p</i> = 0.019). Prehospital infection, thymectomy, Myasthenia Gravis Foundation of America classification and NAR were included to establish the final short-term prognosis prediction model. The good performance of the model was verified by the receiver operating characteristic curve (AUC = 0.782, <i>p</i> &lt; 0.05), calibration curve (chi-square = 8.492, <i>p</i> = 0.387) and decision curve analysis plot. Our study found that NAR can effectively monitor disease severity in patients with MG and is independently associated with short-term prognosis. The nomogram visualizes the prediction model and is beneficial for clinical application.</p>
