<p>Annexin A1 (ANXA1), a calcium-dependent endogenous immunomodulatory phospholipid-binding protein, is ubiquitously expressed across diverse tissues and cell types. Originally identified as an anti-inflammatory protein, subsequent research revealed that ANXA1 exerts a broader influence on cellular processes beyond inflammation, including tumor cell proliferation, apoptosis, invasion, migration, angiogenesis, and multidrug resistance. In gastric cancer (GC), numerous factors contribute to the complex processes of proliferation, invasion, migration and multidrug resistance among which ANXA1 plays critical roles. Understanding the mechanistic role of ANXA1 in GC is crucial for advancing early diagnosis, treatment strategies, and prognosis assessment. This review summarizes the current knowledge on the physiological functions of ANXA1 in GC pathogenesis and evaluates its potential as both a therapeutic biomarker and a diagnostic biomarker.</p>
