摘要

Purpose of Review

This study aims to summarize the latest review research results on obstetrical fistula Globally. The main topics that will be in our interest include demographics and epidemiology, risk factors and consequences, issues on management, and the programmatic progress noted through the global agenda.

Recent Findings

The latest research on obstetrical fistula includes several systematic reviews on treatment and epidemiology. Despite the number of published papers, there is still a gap in research in terms of data of high quality, research methods, and small representativity of countries involved, and within the same country gap in the representativity of the whole population leading to a deep Incidence and prevalence variation emphasizing the relative invisibility of the problem.

Recent findings about incidence and prevalence did not show significant progress compared to the earlier data from the WHO preventing us from having a clear evidence-based estimation of the extent of the problem and its current trends.

Risk factors are still present particularly in rural areas, despite the increasing number of Cesarean Sections, the decision to proceed to CS, when to do it, and how to do it results in many unnecessary cesarian sections without any impact on preventing the fistula occurrence. Instead, there is a continuous rise of iatrogenous fistula requesting more training, capacity building, and supervision of fistula surgeons.

The failed cure rate is higher than the WHO standard, and its associated factors include surgeon expertise and recurrent cases with fibrotic tissue.

The Global Health Agenda will or will not reach the goal of zero new fistula cases by 2030, is still questionable, despite enormous progress made by states and UN organizations while the backlog of cases needs more attention and intensive actions.

Summary

In conclusion, although advancing, obstetrical fistula research presents several gaps in methodology and high-impact trials. Moreover, the mains systematic review published recently refer to results from old data requesting more fresh and newly collected large-scale data.
