Purpose of review <p>Recent advancements in artificial intelligence (AI) applied to coronary computed tomography angiography (CCTA) may refine personalized care by enhancing visualization of atherosclerosis, improving cardiovascular risk stratification and treatment guidance. The present review focuses on AI-guided innovations in CCTA analysis to evaluate atherosclerotic cardiovascular disease (ASCVD), its prognostic utility, and future directions.</p> Recent findings <p>AI-guided CCTA-analysis facilitates automated, reproducible, and scalable quantification of atherosclerosis. A growing body of evidence supports the diagnostic performance of AI-guided plaque quantification on CCTA. These advances support a shift toward more objective and individualized patient care beyond traditional, population-based approaches.</p> Summary <p>As CCTA continues to evolve as an established, first-line diagnostic test to assess ASCVD, a growing body of evidence also supports the diagnostic accuracy and prognostic value of AI-guided CCTA.</p>
