<p>In the German healthcare system, there is a clear distinction between remuneration for inpatient and outpatient care. Billing in the inpatient sector is based on flat rates per case within the framework of diagnosis-related groups (DRG).</p><p>In order to increase existing potential for outpatient treatment for services previously provided on an inpatient basis, so-called hybrid DRG have been defined to enable special sector-equivalent remuneration. Twelve hybrid DRG were defined in 2024, and a further ten hybrid DRGs become available in 2025. However, sleep medicine services have not yet been mapped as a hybrid DRG.</p><p>With this position paper, the German Society for Sleep Research and Sleep Medicine e. V. (DGSM) presents the current service provision and revenue situation for sleep-related breathing disorders as an example. The aim is to create a basis for discussion for the negotiating partners and decision-makers in self-administration and thus to lay the foundation for sector-specific remuneration or hybrid DRG while maintaining safe and high-quality patient care.</p>
