New and groundbreaking insights into auto-immunological processes are currently fundamentally changing our understanding of the pathogenesis of chronic inflammatory central nervous system (CNS) diseases and their available therapeutic options. Given that the integrative function of sleep is gaining importance in the context of individualized patient-focused treatment concepts, this has led to new and specific requirements for somnologists and sleep laboratories.
ObjectivesOverview of the sleep medicine aspects of common (multiple sclerosis, MS) and rare (anti-IgLON5 encephalitis) chronic inflammatory CNS diseases.
Materials and methodsA review of data from a multicenter German survey on sleep behavior in 2052 MS patients and a case series of anti-IgLON5 encephalitis patients are presented.
ResultsOf the MS patients, 73% reported reduced sleep quality, and this, along with fatigue, was the main predictor of impaired quality of life. Furthermore, sleep disorders should not be seen as part of fatigue, but as a separate issue in MS.
In anti-IgLON5 encephalitis, sleep disorders including sleep-wake cycle disturbances and hypersomnic episodes were among the most common symptoms. They were associated with a variety of polysomnographic abnormalities, with higher muscle tone and increased motor activity during REM-sleep facilitating differentiation from other progressive neurodegenerative diseases.
ConclusionsDue to their chronic nature, medication interactions, and the bidirectional interplay between sleep and the immune system, the treatment of sleep disorders in chronic inflammatory neurological autoimmune diseases is gaining in importance.
