Die psychiatrische Abteilung der JVA Neumünster: Symptombelastung und Funktionsniveau der Patienten im Prä-post-Vergleich

Effective approaches to treat mental disorders in prison promise high individual and social benefits, yet there is almost no research on this topic. In this article we report the treatment results from the psychiatric ward at Neumünster Prison.
We conducted a retrospective analysis of data from routine diagnostics at the time of admission and time of discharge. From a total of 414 patients admitted to the psychiatric ward from 2016 to 2024 N = 92 cases were examined. Outcome measures were the self-reported Symptom Checklist (SCL-90-R) and Clinical Outcomes in Routine Evaluations (CORE-OM) as well as an external rating by the main therapist on the Global Assessment of Functioning (GAF).
There were significant improvements in the mental load (SCL-90-R: t = −4.93, p < 0.001, d = −0.52; CORE-OM: t = −7.49, p < 0.001, d = −0.78) and the psychosocial functioning level (GAF: W = 1507, p < 0.001, r = 0.76) between diagnostics made at the time of admission and time of discharge. The changes in the self-reporting measures SCL-90‑R and CORE-OM are strongly positively correlated (r = 0.79, p = <0.001, 95% confidence interval, CI 0.70–0.85), while self-reporting and external rating are not.
Treatment on the psychiatric ward is associated with an improvement in the patients’ self-reported and externally reported mental state. This could indicate effectiveness of treatment in Neumünster Prison, although time effects and special prison conditions on the ward must be considered as influencing factors. Prospective, controlled studies are necessary to confirm these results, determine long-term effects and develop context-sensitive treatment.
