Safety and efficacy of simple system for artificial chordae length adjustment in patients undergoing minimally invasive mitral valve repair

For patients who require mitral valve repair, it is often difficult to determine the appropriate artificial chordae length. The Memo 3D/4D ReChord system, developed for use as a temporary chordal guide system, is comprised of a complete prosthetic ring containing yellow loops that function as a reference for automatically determining neo-chordae height. The present study was conducted to evaluate the safety and efficacy of this system when used with a minimally invasive right mini-thoracotomy approach.
MethodsSixty-three patients (40 males, 64.4 ± 13.3 years old) using this system were enrolled. Preoperative status and operative procedures, as well as early- and mid-term results were assessed.
ResultsThe number of artificial chordae was one in 19, two in 39, and three in 5 cases. In 33 patients, the treatment was completed with use of the ReChord system alone, while 30 required additional procedures. Mitral valve repair was successfully performed in all and there were no mortalities. Postoperative echocardiography mitral regurgitation grade was none in 16 and mild in 47. Two patients required repeat mitral valve surgery. Five- and seven-year survival rates were 93.3% and 90%, respectively, and the rates for freedom from reoperation after five and seven years were 95.3%, and 95.3%, respectively.
ConclusionsMitral valve repair with the Memo 3D/4D ReChord system provides for easy adjustment of the artificial chordae, with acceptable early and mid-term results noted in the present cases. This useful system is considered to be an effective option for a minimally invasive right thoracotomy approach.
