Objective <p>To evaluate the clinical efficacy of Tuina (Chinese therapeutic massage) manipulation on the back points as well as the Yin and Yang Heel Vessels in treating insomnia due to Yin deficiency with fire hyperactivity, and to explore the clinical significance of the meridian theory of directional reinforcing and reducing, and the Yin-Yang theory in guiding Tuina treatment for insomnia.</p> Methods <p>Eighty-eight patients with insomnia due to Yin deficiency with fire hyperactivity meeting the inclusion criteria were recruited and randomly assigned to a trial group and a control group using the random number table method, with 44 patients in each group. The trial group received Tuina manipulations on the back meridian points and the Yin and Yang Heel Vessels three times weekly, while the control group was administered oral eszopiclone tablets as an intervention. Both groups underwent treatment for 4 weeks. Pittsburgh sleep quality index (PSQI) score, traditional Chinese medicine (TCM) symptom score, insomnia severity index (ISI), and sleep structure (sleep duration and deep/light sleep duration) before and after intervention were observed to objectively assess the clinical efficacy of this Tuina method in treating insomnia due to Yin deficiency with fire hyperactivity.</p> Results <p>The total effective rate was 93.0% in the trial group, higher than 90.7% in the control group, but the difference between the two groups was not statistically significant (<i>P</i>&gt;0.05). After treatment, the PSQI and ISI scores declined in both groups (<i>P</i>&lt;0.05); the total sleep duration, deep sleep duration, and light sleep duration improved in both groups (<i>P</i>&lt;0.05). However, no statistically significant differences were observed between the two groups (<i>P</i>&gt;0.05). After treatment, the TCM symptom score declined in both groups, with the trial group scoring lower than the control group (<i>P</i>&lt;0.05).</p> Conclusion <p>Both Tuina on the back points and Heel Vessels and oral administration of eszopiclone improve sleep quality, optimize sleep structure, and reduce insomnia severity in patients with insomnia due to Yin deficiency with fire hyperactivity. Tuina demonstrates superior effects to oral eszopiclone in alleviating TCM symptoms in insomnia patients.</p>

Objective
