<p>Crehan et al. present important data on the safety of robotic cholecystectomy (RC) in a high-comorbidity, medically underserved population. While the findings are encouraging, several methodological aspects merit closer scrutiny. The lack of a laparoscopic comparator group limits conclusions about RC’s relative value. Additionally, variation in surgeon experience was not examined, potentially obscuring its influence on outcomes. The study also does not address whether RC performs consistently across high-risk subgroups, such as patients with sickle cell anemia or cerebrovascular disease. Finally, we support the authors' emphasis on structural barriers to surgical care and call for future studies that integrate both clinical and health systems perspectives.</p>
