Prevention and Management of Recurrent Weight Gain After Bariatric Surgery: Integrating Behavioural, Pharmacological and Procedural Approaches

Recurrent weight gain (RWG) after bariatric surgery leads to a suboptimal clinical response for many patients, challenging long-term outcomes. This narrative review focuses on evidence-based strategies for the prevention and management of this multifactorial challenge. Successful persistent outcomes rely on proactive, structured multidisciplinary follow-up to enable early detection of weight trajectories and timely, tiered intervention. Initial treatment involves intensive nutritional and behavioral therapy, including cognitive behavioral techniques. When insufficient, pharmacological adjuncts, notably GLP-1 receptor agonists, demonstrate efficacy. Endoscopic revision and surgical revision or conversion are reserved for anatomical causes or refractory cases. A tailored integration of behavioral, pharmacological, and procedural strategies is essential to sustain metabolic health and durable weight control.
