Primary Metabolic and Bariatric Surgery in Persons Aged over 65 Years. GRADE-based International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) Position Statement

With the global rise in obesity among individuals aged 65 years and older, there is increasing demand for effective interventions that extend beyond lifestyle or pharmacologic approaches. This IFSO-endorsed Position Statement reviews the role of Metabolic and Bariatric Surgery (MBS) in the elderly population, synthesizing current evidence and expert consensus. Available data show that, when performed in carefully selected patients, MBS in older adults is associated with acceptable morbidity and mortality rates, comparable to those seen in younger cohorts. Significant improvements are consistently observed in weight loss, type 2 diabetes control, hypertension, and functional capacity. The statement emphasizes that advanced chronological age alone should not preclude surgical consideration. Instead, physiological status, frailty, cognitive reserve, and life expectancy should guide multidisciplinary assessment and decision-making. Practical recommendations are provided regarding procedure selection, perioperative management, and long-term follow-up. This document reinforces the role of MBS as a safe and effective option in well-selected elderly patients, expanding access to surgical care in a population often overlooked.
