<p>Internet gaming disorder (IGD) is a behavioral addiction disorder resulting from non-substance use, which has gradually become a widely recognized public mental health issue and has been included in the International Classification of Diseases (11th edition) (ICD-11) by the WHO. Severe addictive behaviors may affect the neurodevelopment of individuals, including abnormalities in brain microstructure. Our aim was to explore alterations in brain white matter microstructure in adolescents with IGD. A systematic search was conducted for relevant studies investigating partial changes in Fractional anisotropy (FA) between individuals with IGD and non-addicted controls, with the search cut-off date set at January 18, 2025. White matter structural changes were meta-analyzed using seed-based d mapping with permutation of subject images (SDM-PSI) based on whole-brain DTI studies. Meta regression analyses were subsequently performed. Six studies met the inclusion criteria, comprising 148 internet gaming addiction subjects and 146 non-internet gaming addiction subjects. Meta-analysis revealed significantly decreased FA values in the genu of the corpus callosum (gCC) in adolescents with IGD. Through meta-regression analysis, we found a negative correlation between the FA value of the gCC in IGD adolescents and their mean age. This meta-analysis identified the most robust white matter microstructural abnormalities in IGD, particularly in the gCC. Moreover, these alterations are correlated with the mean age. These findings highlight the importance of early identification and intervention of this disorder.</p>
