Implantation of an elbow prosthesis in the presence of significant bone defects represents a surgical challenge, as it cannot be performed as a standard procedure. Careful cementing, correct implant selection, and preservation of the extensor apparatus are crucial for successful implantation. To improve primary stability, techniques such as impaction grafting, allograft strut grafting, and the use of an allograft–implant composite are available. The research and expertise on these methods is limited, and the procedures should therefore be considered as salvage procedures and carried out by specialized elbow centers. The main issue remains the high risk of complications, particularly the risk of infection. Alternative treatment methods include resection arthroplasty and ulnohumeral arthrodesis.
