Fractures of the coracoid process are generally rare. They usually occur in the context of high-impact trauma as part of complex scapular injuries. After a dislocation, the coracoid process can also fracture in isolation.
ObjectivesThe aim of the work is to present therapy options and surgical treatment for fractures of the coracoid process.
MethodsAll cases with coracoid fractures from the last 10 years of a level 1 trauma center were included in a retrospective evaluation. A total of 48 cases could be followed up.
ResultsNon-displaced coracoid process fractures distal to the coracoclavicular ligaments are usually treated conservatively. Dislocated fractures proximal to these ligaments or other concomitant injuries of the superior shoulder suspensory complex (SSSC) represent an indication for surgery. A total of 54% of the fractures were distal to the coracoclavicular ligaments. Conservative treatment was carried out in 57% of cases. 43% of the patients examined were treated surgically, 85% of whom underwent screw fixation. 79% of the patients showed concomitant injuries of the ipsilateral shoulder.
ConclusionCoracoid process fractures are complex injuries that cannot be considered in isolation. Existing concomitant injuries are an important factor in the initial treatment concept of the shoulder. The procedure of choice is open reduction and direct screw fixation.
