Neue Hemmstoffe des RAAS für die Blutdrucksenkung

Blockade of the renin-angiotensin-aldosterone system (RAAS) is one of the most important components in the treatment of hypertension. Despite established and effective RAAS blockers, blood pressure control remains inadequate in many hypertensive patients. The reasons for this are multifaceted and include intolerances, side effects, incomplete RAAS blockade, nonadherence and indication limitations in advanced chronic kidney disease. New pharmacological strategies are therefore needed to improve blood pressure reduction through more targeted and longer-lasting effects on the RAAS. Recently, three new approaches to inhibiting the RAAS have become or will become available, which have already been tested in large studies for their effectiveness in the treatment of uncontrolled or treatment-resistant hypertension: small interfering RNA (siRNA) therapeutics against angiotensinogen, nonsteroidal mineralocorticoid receptor antagonists (nsMRA) and aldosterone synthase inhibitors (ASI). In this article we describe the specific mechanisms of action of these various new RAAS inhibitors, the blood pressure reducing effects demonstrated to date and the possible indications. Despite limited study data, the new RAAS inhibitors appear to offer promising prospects for improved blood pressure control, cardiovascular prognosis and renal protection.
